The illegal use I know of personally is from people going across the border to get it.
In the case of the show: It's a small (though with a disproportionately large percentage of upper middle class/lower level rich families) in New England. Main, I think. The character in question, Spencer, first asks a friend to share some of his medication with her. The friend explicitly has a prescription as a study aid, not ADHD, but apparently doesn't habitually share it. Spencer (who is exploiting a relatively mild crush on her) seems to be the exception. She asks him for more and he tells her he's out, so she calls her doctor, claiming to be her mother, and is given a prescription that day with no consultation or known history of ADHD. She runs out of that and apparently stalks the lunchroom, looking for kids taking meds with their lunch. She spots another student she knows literally tossing a few pills down her throat at lunch and approaches her, only to learn that the other girl regularly sells her medication and has multiple and regular repeat customers, and a supposedly legal and endless supply. (For bonus fun, the one who is presented as an actual drug dealer is the only one of the three who isn't white.) The show seems to be under the impression that all teens everywhere do this with no problems.
